Medicaid parity begins with understanding the financing rules

Puerto Rico's Medicaid program does not follow exactly the same federal financing structure as programs in the 50 states and Washington, D.C. CMS's Puerto Rico overview explains the different structure and the additional federal funding Congress has provided at various times. Temporary appropriations can stabilize care, but they also require repeated legislative decisions and planning for their expiration.

The question for a family is whether providers participate and appointments remain available; the question for Congress is how a funding formula and any cap affect the system's ability to plan. Comparing one federal match percentage without the cap, supplemental funds, local contribution, and covered services can mislead. An honest parity proposal should show all of them.

NAPREC advocates full equality through statehood and asks Congress for a clear multiyear Medicaid framework now. Legislators should request comparable, current measures of enrollment, provider access, and financing, then explain how a status transition would work. No one should promise that a legal change automatically creates doctors or appointments without an implementation plan.
