The public record

Senate President Thomas Rivera Schatz filed Senate Bill 1570 to reopen the physician incentive under the Incentives Code, with a 12 percent rate on eligible medical income for 15 years, on the condition that participating doctors treat patients of the Vital government health plan.

NAPREC analysis

According to News is my Business, the bill would remove the June 30, 2022 application deadline, tie eligibility to Health Department findings of shortages by specialty or area, and require the Treasury to identify funding or offsets before issuing decrees, with certification of compliance with PROMESA and the fiscal plan. The report said physician migration has reduced access to care but gave no figures. A separate bill earlier in the year drew fiscal concerns. Local incentives work alongside federal payment rules: sponsors of a bipartisan bill say Medicare Advantage pays about 40 percent less per beneficiary in Puerto Rico than the national average, and federal Medicaid funding for the island is capped. NAPREC supports equal federal health financing as part of any retention strategy.

The statehood connection

NAPREC supports equality through statehood. Puerto Rico’s residents do not have voting senators or a final House floor vote through their Resident Commissioner, and Puerto Rico does not appoint presidential electors. Admission would address that structural representation question. It would not remove the need to legislate program rules, plan a transition, or hold government accountable for results.

A question for public officials

How many physicians would qualify, and what would the incentive cost the General Fund?
